I had to write to say thank you. I didn't have my card for
a week and my dog Lady B had to have an emergency visit and and urgent care
visit to the vet within two days. I saved almost $60.00. She has seizures and
an enlarged heart so I couldn't get insurance for her. I thought there was no
way to save on the great cost of caring for her. I'm telling everyone I know

My Cocker Spaniel Pulli was bitten by a snake. He almost died, my husband would never let

I bought a german shepard and there are problems w/ these
dogs hips and its considered a “pre existing condition” with pet insurance
because apparently all german sheppards will hae hip problems since this is
aflap 25% off disct there are no pre X or forms or hassle ……if the bill is
$200 its $150 w/ Form Pet def a good deal !

I love the convenience of a discount at time of sale;
immediate gratification is wonderful - as opposed to insurance companies
where you have to wait for reimbursement. I am very happy with the new vet
that I found thru PA. I tell everyone that they should join your program.

Just wanted to thank you folks for the job well done. I am a huge animal

It isn’t the exact same as insurance it is a discount. But
the good part is that there is no claim forms, and all pets even sick ones
are ncluded. My cat was really sick and it really helped me pay for
treatments I woundnt have been able to afford. I am so grateful to formulated
pet plan.

I've been with Pet Assure/Formulated for 3 years now. The service has been
excellent. I not only save a considerable amount of money, but I've found a
vet who has been wonderful to my pets. He's caring, concerned and takes his
time with them. I never feel as if I'm being rushed out the door. I've been
so pleased and impressed with the program that 2 years ago I gave my mom a
